SwiftPath protocols are created and vetted through the RoundTable process. These RoundTable meetings are held around the country 3 -5 times per year. They are invitation only where approximately 20 -25 recognized, high volume joint replacement surgeons gather to obtain consensus on specific questions. The Protocols comprise a major part of the standardized SwiftPath Program, and they are deployed to SwiftPath surgeons after they have created their own surgeon-authored guides.

i. As a SwiftPath surgeon, you will be invited to attend the RoundTable meetings throughout the country. RoundTables cover every topic that is relevant to rapid rehabilitation after hip and knee replacement. Literature review and evidence-based medicine are combined with expert opinion using the Delphi Method and anonymous voting. Surgeons hear input from colleagues across the region, and leave with valuable clinical and academic information. SwiftPath focuses on consensus-driven safe and effective delivery of outpatient joint replacement and enhanced recovery.
ii. The SwiftPath Method has been validated through the RoundTable meetings where thought leaders convene to discuss consensus on major issues.
iii. Consensus from the RoundTables adds to the expansive content of the SwiftPath Method which participants and members have access to.
iv. The Program includes a robust information management solution to track patient outcomes and monitor patients in the outpatient setting.

a. The SwiftPath Method® uses a set of tools that have been vetted by the SwiftPath RoundTable Meetings and proven with clinical outcomes. These tools are arranged in a consistent and logical order and provide a master plan that is matched to each patient’s specific circumstances. The Method integrates patient selection, patient education, specific MIS procedures, pain management protocols, home care monitoring, PROs, and other interventions in an ordered process aimed at creating a working plan that is customized to the patient, the institution, and the surgeon.

Surgeon authored patient guide ii. JointCamp presentation iii. Patient registration cards iv. Patient reported outcomes v. Home care monitoring reported outcomes

The Patient Selection Tool is a series of questions that help determine the patient’s pathway:
We are working toward publishing a video with an overview of JointCamp and Patient Registration, but do not have one at this time. When developed, this video will give a broad-brush understanding of how JointCamp is conducted and show the patient registration process. In the meantime, we can conduct a JointCamp instructor training via webinar prior to your first JointCamp.
The SwiftPath® Program is designed and proven to transform the patient’s total joint replacement experience. Our proven models, methods, pathways, and protocols create a total joint replacement program that achieves exceptional results.
Designing, collecting, and reporting on outcomes is a requirement in the current healthcare space, and SwiftPath has compiled a standardized list of PROs meant to satisfy all CJR requirements and more. The SwiftPath Program includes customizable outcome measures based on national standards. SwiftPath combines a robust Patient Engagement Process with its innovative cloud-based databases and dashboards. Providers and administrators get real time access to their quality results and combine clinical and patient-reported outcomes:
The overall structure of Comprehensive Care for Joint Replacement Model (CJR) is as complex as it is all encompassing and demanding. The key points include defined quality metrics, target prices based on blended hospital and regional spending and a retrospective annual reconciliation. For those 800+ hospitals the goal is to decrease costs while increasing quality – leading to healthier patients and communities.
The CJR model forces hospitals and providers to comprehensively understand their entire total joint episode of care. Hospitals will need to address three critical success factors: care redesign implementation, data that can be measured and managed, and the ability to align providers. SwiftPath helps by providing hip and knee replacement protocols that greatly reduce the need for lengthy recovery, skilled nursing facilities, and extended rehabilitation. The protocols have also been shown to reduce hospital acquired complications (HACs) and other costly complications. SwiftPath also provides a comprehensive teaching program that fully engages providers, patients and family—an engagement that naturally leads to better outcomes and happier patients. Just as importantly, patients are engaged in an extremely functional and successful patient reported outcomes platform that helps to build customer loyalty, improve outcomes, and meets the requirements of CJR.
